I received an email from Sapphire Medical and part of it said:
"\[my name\] to send details of medications therapy tried. Once this is received CBMPs can be tried"
How do I do this?

I think they are referring to therapy to use as a medication / previous treatment.
If you have had therapy and it's not on your summary of care then they are asking you to provide proof of therapy, ie an acceptance letter to show you're having treatment or a discharge letter to show you did have treatment.
I have letters from both of my therapy treatments yet they're not listed on my SoC despite being NHS referrals, so if you don't have letters regarding your therapy then contact your therapist and request them.

Not sure if they're asking for your SOC (summary of care) or just a brief informal email about treatments tried.
Best give em a call.

This means to send them details of any medication your GP has prescribed for whatever you're getting the MC for....this could also include alternative treatments such as physio or any form of (for lack of a better word) alternative treatments such as a chiropractor (assuming it's back pain...u get what I mean) 💚💛❤️
